7/27/12	MVA with Entrapment (B- Shift) Approximately 0730hr ,MVA Entrapment assignment, MLK and Fairhill rd Engine Co 41, Hook & Ladder 10, Rescue Co 1, Chief of the 5th Battalion. Crews extricated the body of a deceased adult male, following a crash into a telephone pole that resulted in the car erupting in flames. Another house fire in the southeast (B- Shift) Approximately 0530hr ,Box alarm, 3100 Block of E99. Engine Co's 41 & 36, Hook & Ladder 36, Rescue Co 1, Chief of the 5th Battalion. 7/26/12	Busy shift for Eastside Companies (B- Shift) ,Box alarm, 2400 block of East 57th st. Engine Co's 7 & 26, Hook & Ladder 7, TF13, Chief of the 2nd Battalion. House fire. 2.5 story wood frame. Working fire dispatch: H& L11 (RIT), E41 (Worker) Box alarm, 6800 block of Morgan ave, Engine Co's 11 & 13, Hook & Ladder 11, Rescue Co 3 (Fill in) , Chief of the 2nd Battalion. Engine 11 reported heavy fire showing upon arrival, from first floor of a 2 story wood frame house. Working fire dispatch: H& L10 (RIT), E10 (Worker) Rope rescue assignment, 1300 East 9th. The Federal building. Engine Co's 1 & 13, Hook & Ladder 1, Rescue Co 3, Chief of the 2nd Battalion, and AC1. 4 construction workers, trapped on suspended platform, 8 floors off the ground, during a thunderstorm. The platform swung away from the building, spun 180* and crashed into the building, three separate times! Engine 1 & Truck 1 removed the windows on the 8th floor to rescue the trapped workers. Rescue Co 3 evaluated and transported 2 of the victims. Box alarm, 17300 block of Euclid ave, Engine Co's 31 & 30 (fill in), Hook & Ladder 31, Rescue Co 3, Chief of the 6th Battalion. Car fire extended into a single story used car dealership office trailer. Handled with the box companies. Box alarm, 7403 Canton ave. Engine Co's 11 & 13, Hook & Ladder 11, Rescue Co 1, Chief of the 2nd Battalion. Upon arrival, Engine 11 delcared "working fire". 2.5 story wood frame house. 7/24/12	House Fire on Gertrude ave (C- Shift) Box alarm, 6800 block of Gertrude ave. Engine Co's 11 & TTF13, Hook & Ladder 11, Rescue Co 1, Chief of the 2nd Battalion. House fire. Working fire dispatch: H&L7 (RIT), E26(Worker), & AC1 ,Box alarm, 4100 block of Bradley rd. Engine Co's 20 & 42, Hook & Ladder 20, Rescue Co 2, Chief of the 4th Battalion. An machine used to produce ammunition, caught fire, sending at least one victim to the hospital with a burn injury. Engine 20 laid out and pumped. Working fire dispatch: H& L 4 (RIT), E24 (Worker)
7/20/12	Engine Co 26 first due! (B- Shift) Approximately 1600hrs, Box alarm, 2500 block of East 82nd. Engine Co's 26 & 7, Hook & Ladder 7, Rescue Co1, Chief of the 2nd Battalion. E26 reported smoke showing from a 2 story wood frame house. Fire in the rear of the second floor. Working fire dispatch: H& L10 (RIT), E10 (Worker)
7/19/12	House Fire on Arbor rd (C- Shift) 0012hrs, Box alarm, 458 Arbor rd Engine Co's 31 & 30, Hook & Ladder 31, Rescue Co 3, Chief of the 6th Battalion. E31 reported smoke showing from a 2.5 story wood frame house. The Fire Alarm Office recieved numerous phone call and started the RIT and Worker along with the box assignment. 7/18/12	Apt fire on Lakeshore blvd (C- Shift) 0925hrs, Box alarm, 15500 Lakeshore blvd Engine Co's 31 & 40, Hook & Ladder 31, Rescue Co 3, Chief of the 6th Battalion Small fire inside an apartment of a Four Suiter, type apartment building.
7/14/12	MVA with Entrapment on Lakeshore blvd (A- Shift) Approximately 0330hrs, MVA assignment, 13000 Lakeshore blvd Engine Co 31, Hook & Ladder 31, Rescue Co 3, Chief of the 6th Battalion. Crews used the Jaws of Life hydraulic tools to extricate a trapped occupant of a late model, "Hummer H1". The vehicle took out a telephone pole, fire hydrant and ended up crashing into a large tree.

E30 reported fire showing from the 1st and 2nd floors of a 2 story taxpayer building. The Fire Alarm Office recieved numerous phone call and started the RIT and Worker along with the box assignment. 7/3/12	House fire in Battalion 4 (B- Shift) 0210hrs, Box alarm, West 58th & Side ave. Engine Co's 26 (Fill in) & 4, Hook & Ladder 4, Rescue Co 2, Chief of the 4th Battalion. Ladder 4 was returning to quarters from another alarm and came upon a 2.5 story wood frame house on fire. E26 was returning to quarters from an alarm in the area also, and was dispatched as first due pumper. (E26 was filling in at 20's house during the prior 2-2-2) Working fire dispatch: H&L 1 (RIT), E24 (Worker) House fire in Battalion 4 (B- Shift) Approximately 0000hrs, Box alarm, 3194 West 54.
